Client Success Stories
We believe in showing, not telling.
So, here are a few of our client success stories.
Planning for Success as a Permanent Supportive Housing Provider
From the Apple Health and Homes Capacity Building grant from the Washington State Department of Commerce, Atrómitos produced a comprehensive toolkit on permanent supportive housing (PSH), which provides a wealth of resources, strategies, and information to support the successful implementation of PSH initiatives.
From Strengths to Weaknesses: A Strategic Journey
To prepare for upcoming policy and funding changes, CINC’s Executive Director sought to conduct an organizational assessment and stakeholder engagement to develop a strategic plan. Atrómitos conducted a SWOT analysis to identify CINC’s strengths, weaknesses, opportunities, and threats and collaborated with the CINC team to develop a strategy map and action plan framework.
Washington Benefit Exchange: The Immigrant Community Health Landscape
CHNW, IRHA, Atrómitos, and MCG Consulting scanned WA’s immigrant communities to guide WAHBE’s outreach for expanded health coverage. WAHBE must understand the health needs of immigrants and use appropriate strategies to ensure they can access coverage.
Sustain the Life of the National DPP
Previously, ADCES engaged us to develop resources and training material to help CHCs integrate the National DPP into their overall model of care. ADCES sought to expand on this work by assisting CHCs to move from integrating the National DPP to finding ways to financially sustain the National DPP lifestyle change program beyond the life of their CDC cooperative agreement.
Focusing on Internal Capacity Development
We can’t overstress the importance of information when it comes to providing the right care, for the right patient, at the right time and place. But ensuring that an organization has the right (and interoperable) technology tools and platforms is only the first part of the problem.
Adapting to Rapid Growth
CINC has experienced significant growth in recent years, driven by the increasing demand from the opioid crisis and the state’s mental health resource challenges. Alongside expanding support services statewide, CINC has adapted by assessing its internal capacity to ensure its operations, governance, and controls align with current contract requirements.
Tackling the Digital Equity Gap in Washington
The Community Health Network of Washington (CHNW) partnered with Atrómitos to develop and implement a Digital Navigator Program to address the Digital Divide, a social health determinant. The program aimed to provide trusted guides to assist community members in internet adoption and the use of computing devices and to advance digital literacy skills, digital equity, and connectivity in Washington.
Starting on the Right Foot: Ensuring a Fair and Accurate Payer Contract
Bladen County Health Department serves as an essential public health provider to nearly 40,000 residents in southeastern North Carolina. Health Department leadership led and organized the county’s response to the public health crisis while negotiating complex contracts with Medicaid Prepaid Health Plans (“PHPs”) and establishing the operational foundations for a sustainable future. Atrómitos brought its A-game.
Increasing Coverage of the National DPP Lifestyle Change Program
Recognizing the CHCs they work with needed additional support with integrating the National DPP into their operations, including understanding how to leverage their reimbursement schedule to sustain the infrastructure necessary to sustain the program, ADCES sought a consultant with expertise in billing for CHCs.
Crafting a Business Plan that Increases Revenue and Drives Organizational Sustainability
El Futuro, a nonprofit outpatient clinic in North Carolina, provides comprehensive mental health services for Latino families and supports the La Mesita Mental Health Professional Network. With the continued growth of La Mesita membership and increased participation in its varied services, El Futuro recognized a need to identify new revenue sources to sustain La Mesita’s operations. Here’s what happened.
Leveraging High Quality and Innovation to Obtain Higher Reimbursement Rates
AssistedCare NC realized that to expand its footprint outside of Southeastern NC and continue to achieve high standards in its processes and outcomes for patients, it would need to invest in its care delivery. Medicare Advantage plan contractors were ignoring requests for rate increases year-over-year, so AssistedCare needed a new approach to negotiating with those contractors. That’s where we came in.
Training Community Coalitions to Adopt an Entrepreneurial Mindset for Sustainability
Community Impact NC (CINC) provides critical training support to community coalitions throughout North Carolina that work to prevent substance misuse and build stronger and safer communities. In 2020, CINC saw that those coalitions needed to make internal capabilities to ensure sustainability and sought to collaborate with an organization specializing in this training type. Enter Atrómitos.
Navigating Medicaid Transformation: A Value-Based Care Project for Tri-County Health Departments in North Carolina
The University of NC Wilmington Center for Healthy Communities sought to maximize the grant dollars received to support the three county health departments in successfully preparing for and engaging in the Medicaid Transformation in North Carolina.
