Association of Diabetes Care & Education Specialists

The National Diabetes Prevention Program (National DPP) lifestyle change program is a structured, evidence-based, year-long lifestyle change program to prevent or delay the onset of type 2 diabetes in adults with prediabetes or at risk of developing type 2 diabetes. Since 2012, the Association of Diabetes Care & Education Specialists (ADCES) has been supporting providers and communities across the country with implementing the National DPP lifestyle change program.

In 2017, ADCES began focusing on bringing the program to communities with little or no access to CDC-recognized programming, prioritizing supporting Federally Qualified Health Centers (FQHCs) and FQHC Look-Alikes (LALs) with implementing the program.

FQHCs and LALs face unique challenges in implementing the National DPP lifestyle change program. To best support them, ADCES contracted Atrómitos and our Advisor, Evolve Health Strategists (EHS), to research their barriers to implementation and develop resources to help them think through ways to sustainably implement the lifestyle change program.

Atrómitos and EHS facilitated training for FQHCs and LALs on the benefits of implementing the National DPP lifestyle change program. The training also covered operational factors FQHCs and LALs should consider to effectively and sustainably implement the program.
